The Illinois Premise Alert Program (Public Act 96-0788) allows individuals with special needs or disabilities, or their guardians, to voluntarily provide information about a person's physical, developmental, behavioral, or emotional conditions that may require special consideration during an emergency. Submitting information is free of charge, and information can be accessed by police, fire, and EMS personnel before they arrive on the scene, helping them to respond more effectively.

What Information Can Be Provided?

  • Type of medical conditions or disabilities, such as Alzheimer's disease, autism, hearing or vision impairments, mobility limitations, mental or emotional conditions, etc.

  • Precautions or special considerations, including communication needs, behavioral triggers, the use of mobility aids or other medical equipment, or other information you think may help first responders provide assistance.

  • Emergency contact information for a family member, caregiver, or medical professional familiar with the individual's needs.

How Is the Information Used?

  • Lake Consolidated Emergency Communications (LAKECOMM) stores the provided information in a secure database. The information is confidential and shared only with police, fire, and EMS personnel through the computer-aided dispatch (CAD) system before they arrive on scene.

  • Note: The information you provide expires 2 years after submission. To ensure the information stays in the database, submit a new or updated form before the expiration date. Additionally, changes to information can be submitted any time during the 2-year period.
